Weighted Vest Walking in 2026: 150 People Wore One 7 Hours a Day for a Year and Still Lost Hip Bone – So What Is It Actually Good For?
For a year, 150 people in North Carolina wore a weighted vest for about seven hours a day. Not for a 30-minute walk. Seven hours, every day, while they lost weight, and with the vest topped up an eighth of a pound at a time so it replaced roughly 78% of whatever they’d shed. At the end of the twelve months their hip bone density had dropped by the same amount as the group who’d worn nothing at all. If you’ve bought into weighted vest walking this year because someone on Instagram told you it would protect your bones through menopause, that trial is the one you need to know about. And it isn’t the only one.

Weighted vests are having a proper moment in Britain this September. They’re on the school run, on the Sunday hill walk, on the 6.45am Hyde Park loop. You can pick up a 10kg one for under £30. Omorpho, the American brand behind the sleek vest you’ve seen on people who also own a Whoop, launched in the UK in June. The pitch is nearly always the same: walking is fine, but walking with load builds bone, burns more fat and builds muscle, and it’s especially important for women over 45.
Some of that is true. A surprising amount of it isn’t, or at least hasn’t been shown. Here’s what the trials actually say, and what I’d do with one if I were buying.
The trial that was meant to settle it
The North Carolina study is called INVEST in Bone Health, run by Kristen Beavers at Wake Forest and published in JAMA Network Open in June 2025. It’s one of the largest weighted vest trials ever funded, with a $2.9 million grant from the US National Institute on Aging behind it. That matters because most weighted vest research before it was small – a few dozen people, sometimes fewer than ten.
The design was clean. 150 adults, average age 66, all with obesity, all put on a weight-loss programme. One third lost weight and did nothing else. One third lost weight and wore the vest daily. One third lost weight and did supervised resistance training. Everyone lost a similar amount – between 9% and 11% of body weight – and adherence was high. The vest group averaged 7.1 hours a day in it.
The result: all three groups lost hip bone density at a similar, significant rate.
Beavers, to her credit, has been blunt about this. Her line in the university’s own write-up was that “these strategies alone may not be enough”. She also said, and this is the bit that gets left out when the study is quoted on TikTok, that the findings are “in no way a reason to stop using weighted vests in exercise regimens”. More than half the participants said they didn’t mind wearing it, and the vest group did lose weight successfully. The vest didn’t fail as a fitness tool. It failed at the one specific job it was being sold for: stopping bone loss.
Why? The honest answer is nobody knows yet. Beavers has secondary papers coming. The plausible explanation is that bone responds to sudden, sharp loading – impact, jumping, heavy lifting – far more than it responds to a steady extra 5kg spread across your shoulders for hours. Your skeleton adapts to what surprises it. A vest worn all day stops being a surprise by about 10am.

Nine trials, 368 people: what weighted vest walking does and doesn’t do
In 2026, a group of researchers pulled together every randomised controlled trial on weighted vests in middle-aged and older adults they could find – nine of them, 368 participants in total, searched up to March this year – and published a meta-analysis in Osteoporosis International. It’s the best summary of the evidence that exists, and it’s a lot more useful than any single influencer’s before-and-after.
Two findings stand out.
First, wearing a vest during exercise beat doing nothing. Compared with people who carried on with their usual activity, the vest-plus-exercise groups had better bone density at the femoral neck (the bit of your hip that breaks) and the lumbar spine, and lost 2.67kg more body fat on average. The fat-loss finding was graded high certainty. The bone findings were graded low to moderate, which in plain terms means “probably real, but we’d like more data before we’d bet the house”.
Second, and this is the one to remember: when the vest was compared against the same exercise without a vest, the extra bone benefit vanished. The vest added about 0.73kg of extra fat loss, which is something. But no measurable skeletal bonus. The authors are careful to say that comparison rests on a single study. It’s still the only head-to-head we have, and it went the wrong way for the marketing.
So the meta-analysis and INVEST tell the same story from two directions. Exercise builds bone. Adding weight to your torso while you do it makes you burn a bit more and might make you a bit stronger. It hasn’t been shown to make your bones any denser than the exercise would’ve on its own.
The menopause pitch, and the number that’s actually true
Almost every weighted vest ad aimed at British women right now leans on bone. The pitch goes: you lose bone fast after menopause, walking with load fixes that, buy the vest.
The first half is correct. Beavers puts normal adult bone loss at around half a percent to 1% a year from your 40s, and it speeds up sharply in the years around menopause as oestrogen drops. The Royal Osteoporosis Society estimates around 3.5 million people in the UK have osteoporosis, and it’s overwhelmingly a women’s problem – on its figures, one in two women over 50 will break a bone because of it. Those numbers are real, and worth taking seriously well before 50.
But the vest-as-solution part is a leap the evidence doesn’t make. And here’s my contrarian position, for what it’s worth: the weighted vest has become a way for a lot of women to feel like they’re doing strength training without doing strength training. It’s the fitness equivalent of buying the expensive running shoes instead of running. If your goal is bone, the things that have repeatedly held up in trials are progressive resistance work (squats, deadlifts, step-ups, loaded carries, going heavier over time) and impact – hopping, skipping, jumping off a step. The NHS guidance for adults is at least two strength sessions a week on top of your 150 minutes of moderate activity, and a vest on a walk doesn’t count as either in any meaningful sense.
A vest can be part of that. Beavers’ own follow-on trials use one, but for stair climbing, box steps, heel drops and single-leg hops – not for a stroll to Tesco. That’s the difference between load and impact, and it’s the whole argument.

Where the vest earns its money
None of this means don’t buy one. It means buy it for the right reasons.
The fat-loss effect is the most solid thing in the whole literature. High-certainty evidence, consistent across trials, and it makes physical sense – you’re carrying more mass, so every step costs more. If you’re already walking daily and want the same route to do slightly more, a vest is the cheapest upgrade available. Cheaper than a gym membership, cheaper than a walking pad, and unlike both you’ll actually use it because it lives by the front door.
The strength and balance findings in older adults are decent too. Earlier trials from the early 2000s – the ones INVEST was built on – found improvements in leg strength, sit-to-stand performance and balance when older people exercised in a vest. Sit-to-stand is one of the best predictors of who ends up needing care, so that isn’t a trivial outcome. For someone in their 70s who won’t go near a barbell, a 3kg vest on the daily walk plus a few step-ups on the bottom stair is a reasonable, low-drama intervention.
And there’s the boring benefit, which is that hill walks get harder. I borrowed my sister’s 8kg one in August for a fortnight of Peak District walks. The first day up Mam Tor with it on, I stopped twice where I’d normally stop once. By the second week I’d stopped noticing it on the flat. Whether that’s fitness or just getting used to it, I can’t tell you. But I did want to keep going, which is more than I can say for most kit I’ve tried.
What I’d actually do with one
If you’ve got a vest or you’re about to get one, this is how I’d use it based on what the trials reward.
Start at 5% of your body weight and don’t go past 10% for walking. Most of the trials used somewhere in that range, and the ones that went heavier were doing it under supervision. A 70kg woman is looking at 3.5kg to 7kg, which is a lot less than the 10kg and 15kg vests that dominate the bestseller lists.
Put it on for the hard bits, not the whole day. INVEST’s seven-hours-a-day regime didn’t protect bone; there’s no reason to think a sedentary nine-to-five in a vest would do better. Thirty to forty-five minutes, three to five times a week, during actual movement – that’s the pattern most of the positive trials share.
Find some stairs. Or a steep bit. Or a kerb to step up and down. Flat pavement walking in a vest is mostly a cardio and calorie story. The moment you add a step, you add the kind of loading that bone actually notices. Keep lifting, or start. The vest is an add-on to strength training, not a replacement. Two sessions a week with something heavy in your hands does more for your bones than any amount of walking with something heavy on your back. Who should leave it on the shelf
Anyone with diagnosed osteoporosis, above all anyone with a history of spinal compression fractures, should talk to whoever manages their bone health before strapping weight to their spine. This isn’t box-ticking caution. Loading a fragile spine vertically is precisely the mechanism behind compression fractures, and the trials that found benefits were run in people who were generally healthy.
If you’ve got a dodgy lower back, neck problems or a shoulder that clicks, the vest will find it. The weight sits on your traps and upper spine and the cheaper ones distribute it badly. Two people I know gave up within a week for that reason alone.
Runners should be careful too. The vest brands love a running shot, but adding 10% to your body weight on a run adds a lot more than 10% to the force through your knees and shins, because impact forces scale with speed. Weighted vest walking has a decent evidence base. Weighted vest running has an injury-risk profile and not much else. If you’re training for a 10k this autumn, do your running without it and use the vest on your recovery walks. The zone 2 training crowd will already know why: the point of easy miles is that they’re easy.
And if you’re pregnant, or you’ve just started exercising after a long gap, this is a year-two tool, not a week-one tool. Get the walking habit first. The Japanese walking method – three minutes hard, three minutes easy – does more for a beginner than any amount of extra load and costs nothing.
£25 or £150: does the price matter?
Less than you’d think. The trials didn’t use anything fancy – most used simple adjustable vests with pockets for iron bars or sandbags, the sort you’ll find on Amazon for £30 to £50. What you’re paying for at the top end is comfort: a snugger fit, weight spread across the whole torso rather than hung off your shoulders, no bounce, materials that don’t smell of rubber for the first month.
For walking, that comfort matters more than it would for a gym session, because you’re in it for longer. A vest that rubs or shifts won’t get worn, and an unworn vest has a bone-density effect of exactly zero. My rough rule: if you’re going to wear it more than three times a week, spend £60 to £90 on something adjustable in 0.5kg or 1kg increments with a proper chest strap. If you’re not sure you’ll stick with it, borrow one or buy the cheap one and see if you’re still using it in November.
Skip anything that isn’t adjustable. A fixed 10kg vest is too heavy for most women to start with and too light for most men to progress to, so it ends up in the cupboard with the resistance bands and the ab roller.
The verdict
Weighted vest walking is a good way to make a walk harder, burn a bit more, and add some strength and balance work into a routine that wouldn’t otherwise have any. That’s a real benefit and I’m not going to pretend otherwise.
But the bone-building claim that’s selling most of these vests in Britain this autumn is running well ahead of the evidence. The biggest trial found no protection at all, and the best summary of the rest found no bone benefit beyond what the exercise itself was already doing. If bone is your reason, the vest belongs on your back while you climb stairs, do step-ups and lift things – not on a flat 20-minute walk with the dog.
