
Vitamin D in Winter 2026: A 25,871-Person Trial Flopped, the NHS Still Says Take It From October – and Both Are Right
Britain’s supplement aisle has a blind spot you could park a bus in. We’ll pay £45 for a frosted jar of bovine colostrum on the strength of a TikTok, while the one supplement that carries an actual government recommendation – vitamin D in winter, ten micrograms a day from October to March – costs about as much per month as a meal deal and mostly goes untaken. Nobody films themselves taking it. There’s no frosted jar. And in a market that runs on novelty, that seems to be precisely its problem.

The science looks like a mess from the outside, too. The biggest trial ever run on the vitamin came back empty. So did the second biggest.
Yet both things are true at once: the big trials flopped, and the advice to take it still stands. Working out how that can possibly be the case takes about ten minutes, which is roughly nine and a half more than the supplement industry would like you to spend on it – because the answer leads you to a tablet that costs 3p, not a subscription.
How a cheap vitamin got hyped into a cure-all
To understand why the big trials felt like such a betrayal, you have to remember what came before them. Through the 2000s and 2010s, observational studies kept finding that people with low vitamin D levels had more of almost everything: more heart disease, more cancer, more depression, more multiple sclerosis, more dementia. Hundreds of papers, one direction. The vitamin started to look less like a nutrient and more like a master key, and supplement sales moved accordingly.
The catch sits in the word ‘observational’. Those studies could show that low vitamin D and poor health travel together; they couldn’t show which one was driving. And there’s an obvious third passenger. People who are unwell, elderly, obese or housebound spend less time outdoors in the sun – so illness lowers vitamin D at least as plausibly as vitamin D prevents illness. Measured this way, a low blood level behaves less like a cause of disease and more like a marker of a life spent indoors.
The pandemic gave the hype one last lap of honour, with vitamin D briefly recast as a COVID shield. NICE reviewed that evidence in late 2020, at the peak of the excitement, and found nothing solid enough to recommend it for prevention or treatment – just the same boring winter advice as before.
Randomised trials exist to settle exactly this kind of argument. So researchers ran some very big ones.
The decade vitamin D kept failing its exams
In late 2018, Harvard researchers unveiled the results of a study called VITAL, the largest randomised trial ever conducted on vitamin D. It gave 25,871 American adults either 2,000 IU of vitamin D3 a day – five times the dose the NHS suggests – or a placebo, and followed them for a median of 5.3 years to see whether the supplement reduced invasive cancer or major cardiovascular events. It didn’t. Not by any margin the statisticians could stand behind.
The follow-ups made things worse. A 2022 analysis of the same trial logged just under 2,000 fractures among participants and found the vitamin D group broke no fewer bones than the placebo group. Australia’s D-Health trial gave 21,315 people a hefty monthly dose for five years and found no reduction in deaths. One by one, the grand claims – heart protection, cancer prevention, stronger bones for all – failed to survive contact with a control group.
By about 2023 the verdict had hardened into received wisdom. Vitamin D was the decade’s most overrated pill, and the headlines said so.

You can’t fill a tank that’s already full
Look at who was actually in those trials, though, and the story changes shape. VITAL didn’t recruit people who were short of vitamin D. It recruited ordinary American adults, most of whom started the trial with perfectly healthy blood levels, many of them living at latitudes where winter sun still does something useful. The trial then asked whether pouring more vitamin D into replete bodies produced extra benefits. The answer was no – which is what the answer usually is when you top up a tank that’s already full and check whether the car goes faster.
A vitamin isn’t a drug. A statin lowers cholesterol in more or less anyone; a vitamin only fixes the problem of not having enough of that vitamin. Past the point of sufficiency, the body has no use for the surplus. The big trials proved that extra vitamin D does little for people who already have enough, and almost nothing about what it does for people who don’t.
Those are different questions. And the second one is where Britain comes in.
What actually happens to vitamin D in winter in Britain
Your skin makes vitamin D when a specific band of ultraviolet light – UVB – lands on it. From about October to early March, the sun over the UK never climbs high enough in the sky for meaningful amounts of UVB to get through, a point the NHS makes plainly: between those months, most of us simply can’t make vitamin D from sunlight at all, whatever the weather is doing. A bright blue January day in Leeds produces roughly the same amount as a cellar. Which means the skin on your forearms is running its last proper production shifts of the year right about now.
Food doesn’t rescue the situation either. Vitamin D turns up in oily fish like salmon, mackerel and sardines, in egg yolks, in red meat, and in fortified breakfast cereals – but in amounts that make the ten-microgram daily target genuinely hard to reach from a normal diet. The government’s nutrition committee looked at this in detail in 2016 and concluded as much, which is why its advice reached for a supplement rather than a food list.
The result is visible in the population data. Around one in six UK adults has winter blood levels below the recommended threshold. Rickets, a disease with a strong claim to being the most Victorian thing still in circulation, has never quite left the English hospital statistics.
But notice what kind of claim this is. Correcting a seasonal shortfall in a country sitting at 50-plus degrees north is the one job vitamin D demonstrably does – the unglamorous, undisputed, load-bearing use case that keeps bones mineralised and muscles working. It’s also the only claim that never trends.

The 3p tablet and the ‘high strength’ problem
The official advice is almost insultingly modest. Ten micrograms – 400 IU – each day from October to the end of March, for everyone over the age of four. A 90-tablet tub of supermarket own-brand vitamin D3 sits at the back of the vitamin shelf for somewhere between £1.50 and £2.50, which works out at roughly 3p a day. An entire winter of doing the recommended thing costs less than one oat flat white. Pick D3 over D2 if you have the choice; it’s the form your skin makes and it’s a bit better at raising blood levels.
The industry can’t make money from that, so it has quietly moved the goalposts. Walk into any high-street health shop and the shelf pulls your eye past the sensible dose towards ‘high strength’ 1,000 IU, then ‘ultra’ 4,000 IU, at five or six times the price. What the label doesn’t mention is that 100 micrograms – that same 4,000 IU – is the NHS’s safe upper limit for adults, not a target. It’s the ceiling, sold as the product.
Vitamin D is fat-soluble. It accumulates. Take grams of vitamin C you don’t need and you produce expensive urine; run high-dose vitamin D for long enough and you can push calcium to levels that damage the kidneys. A 2022 case report in the BMJ’s journals described a British man who spent more than a week in hospital after a supplement regime that included vitamin D at hundreds of times the daily amount. He is an outlier, but he is the logical end point of a shelf that prices doses like ambition.
High-strength vitamin D for the general public is, frankly, the laziest upsell in the aisle – the same trick that turns a 3p multivitamin into a £97 pouch of greens powder. More isn’t better here. More is just more, until it’s worse.
The winter mood claim – mostly a miss
The claim you’ll hear most between now and Christmas is the mood one: that vitamin D is the fix for the British winter gloom. It’s an appealing idea. The sun goes, the vitamin goes, the mood goes – surely one tablet reconnects the circuit.
The evidence declines to cooperate. The same VITAL programme ran a dedicated depression arm in more than 18,000 older adults and found the supplement made no difference to whether people became depressed or how their mood tracked over five years. Smaller trials on seasonal low mood point in various directions, none convincingly. If winter flattens you, the interventions with actual evidence behind them are light, exercise and talking therapies – and if it’s more than flattening, that’s your GP, not the vitamin shelf.
Take the tablet for your bones and muscles. Just don’t expect it to do the work of a lamp, a run or a conversation.

Skip the blood test
The wellness industry’s other answer to the awkward trial data is measurement. Finger-prick vitamin D kits run £30 to £60, and full screening packages will happily fold your levels into a £299 morning of preventative theatre.
For most people the number changes nothing. If your level comes back fine, ten micrograms through winter is still sensible and still safe. If it comes back a bit low, the remedy is the same tablet. NICE doesn’t recommend routine testing for people without symptoms or risk factors for exactly this reason: the result rarely alters the action. Genuine symptoms – bone pain, persistent muscle weakness, a baby or young child you’re worried about – are a GP conversation, not a mail-order data hobby.
Put the £40 towards the next thirteen winters of tablets instead. That’s not a figure of speech; that’s the actual arithmetic.
Who should start in October – and who shouldn’t wait that long
For most adults, the NHS position is refreshingly binary. April to September, your skin plus an ordinary amount of time outdoors does the job, and you can stop paying attention – the sensible sun habits covered in our look at what British summers do to skin still apply. October to March, take the ten micrograms.
Some people shouldn’t stop in spring, though. The NHS advises a year-round supplement if you have darker skin, since melanin slows vitamin D synthesis and a British summer may not compensate; if you’re rarely outdoors, housebound or living in a care home; if you usually cover most of your skin outside; or if you’re breastfeeding a baby, who needs drops of their own from birth. Children aged one to four are advised to take it daily all year as well – and families on Healthy Start can get those vitamins free, one of the quieter bargains in British public health.
I put my own tub back next to the kettle last week, mostly because starting in August means the habit exists by the time it matters. Left to its official start date, the ritual tends to dissolve somewhere between half-term and the first mince pie – the same drawer-of-good-intentions fate that swallows most of Britain’s magnesium by February.
The clocks go back on 25 October. The trials will keep arguing about what vitamin D can’t do, and the aisle will keep inventing strengths nobody asked for. In between sits a 3p tablet doing the one thing everyone agrees on.
